Please note that a "nurse profile" must be completed with US NurseLink prior to applying for any shifts.  Your profile (Packet 1 Documentation) includes a nurse application, 2 professional references and an appropriate skills checklist for the shift you are applying for.  If you would like to complete your profile now, click on the links below, complete the forms and fax them to US NurseLink toll free at 866-945-4895.  Your profile will be uploaded into our system as qualified to respond to auto notifications for open positions once we receive your documentation.  

The facilities that have posted the open position will respond to your profile submission via email in one of three way:  Acceptance, Request for Interview, or Declined.  If Accepted, the shift will be confirmed and booked as soon as the nurse completed Packet 2 documentation with US NurseLink.  There may be several nurses accepted for the same open position and the first nurse to have their Packet 2 documenation complete will be able to confirm the shift and be guaranteed the shift offered.
